The mission of Community Health Care, Inc. is to provide the communities we serve with excellence in patient-centered medical, dental and behavioral health care that is compassionate, affordable and accessible.  Over the past 47 years, CHC has grown to be one of the largest outpatient health care systems in the region.  We currently operate 10 clinic locations serving more than 40,000 individual patients throughout Muscatine, Clinton, and the Quad Cities.  The CHC Muscatine Medical & Dental Clinics opened in 2020, providing family medical, dental, telehealth and behavioral health services.  In addition to those services, our Community Health Worker and CHC Outreach Programs connect community members to many other social services and resources including; housing assistance, child care, food, clothing and more.  For appointments, please call our CHC Muscatine Clinics at (563) 327-2140.  For more info on our CHC services and locations, visit http://www.chcqca.org or call our central scheduling office at (563) 336-3000.
